Reference Policy for Changes in Qualifiers

When the qualifier of a body changes, e.g. (Moscow, R.S.F.S.R.) to Moscow (Russia) always make the ‡wnne reference, even though it may seem incorrect because the former heading is no longer a valid AACR2 reference (RI24.4C6)
