EMERGENCY INFORMATION
INJURY AT WORK
1. Injured graduate student employees must go to either the IU Health Center or Bloomington Hospital Emergency Room. These are the only two emergency services covered by IU insurance.
2. Obtain an Accident Request form from David Grindle in A203D.
3. The form must be completed in its entirety (incomplete forms will be returned) and returned to the David Grindle as soon as possible. All Health Center and Bloomington Hospital invoices should be submitted as soon as possible to the Executive Secretary.
IU VEHICLE
In the event of an accident involving an IU vehicle, contact, in this order:
Monroe Co. Brown County/BCP
1. AMBULANCE 911 911
2. POLICE ( Bloomington ) 339-4477
Indiana University 855-4111
3. IU MOTOR POOL Regardless of where accident occurs, call 855-3300; if no answer, call 855-9514. They will tow the vehicle.
4. DAVID GRINDLE 855-7303 or 323-8670. If no answer call Jonathan at 855-4502 or 334-3097.
If the accident results in injury, also contact one of the following administrators within our department (if not already contacted):

REMEMBER:
ALL ACCIDENTS AND DAMAGE TO THE VAN, NO MATTER HOW SMALL, MUST BE REPORTED TO DAVID GRINDLE . IF THEY ARE NOT REPORTED, THE UNIVERSITY'S INSURANCE DOES NOT COVER THE DAMAGE AND THE DEPARTMENT OF THEATRE AND DRAMA MUST PAY FOR THE REPAIRS. THANK YOU!
