name, marital status, social security number, birth date, ethnicity,
and home address-W2.

to identify veteran status, prior work experience, professional education,
licenses and certifications, emergency contact information, and honors
and awards.

This form should be completed by the employee
if wanting to be identified as a person with a disability and desiring
to be considered under IU’s affirmative action plan. Information
provided is kept confidential in accordance with the Americans with
