A215 Review Session Change Request Form

To request a change of Review Session in the A215 Lab, complete and submit the request form below.

You will receive an email summary of your request.

You will be notified by a separate email from the A215 Lab Director when your request is approved

You may NOT attend a review other than your own section's without prior approval from the A215 Lab Director!

11:15am Friday - 11:00pm Sunday For Friday, Saturday and Sunday, indicate all other times when your schedule would prevent you from attending another section's review because of other commitments. Again, please specify reason for each time conflict. [WARNING: detailed reason(s) must be given for each time conflict, otherwise request will be disregarded]. For each day on which you have no time conflicts, enter "None." (Note: there are no Thursday reviews scheduled for the current series of review sessions.)
