A $2o.oo per half-hour fee will be charged for programming scanner to read
more complex forms.

MULTI-OP COURSE EVALUATIONS
Indiana University – Bloomington Campus: Multi-Op forms are provided
free of charge for all IU-B classes. However, the MAXI duplicating charges will
be incurred by the department. Please provide an account number with your
Multi-Op order for this purpose.
