Please be sure to make the “Study Information Sheet” available to students. One copy is enclosed with this shipment, and a PDF of the document was attached to the e-mail notifying you that your surveys were shipped from our office. Teachers/proctors should read through the “Information for Teachers” and “Survey Administration Instructions” before administering the survey in order to get familiar with the survey and the administration process. When students finish filling out the survey, have them put their surveys directly into the “Survey Return Envelope.” Teachers/proctors should then seal the envelope and return it to you or another designated HSSSE contact person. When all surveys are completed and returned to you in the “Survey Return Envelopes,” pack the envelopes in these boxes (or any available boxes), seal them, affix the shipping labels to them, and have them picked up by FedEx (1-800-GO-FEDEX). Do not send us partial shipments of surveys; only ship your surveys back to us once, when you have all of your surveys together. That way, we can be sure that your data are compiled cleanly and accurately. Please have FedEx pick up your completed surveys as soon as they are ready for shipment, and e-mail us when you have shipped your surveys to let us know the number of boxes we should expect. Please do not print and/or distribute materials from the HSSSE website; you will receive enough copies for the population you intend to survey and those administering the survey after you have registered. |
