instructor’s approval. Examines common tools and techniques for
evaluation and injury prevention. Analysis of factors leading to
unintended catastrophes in the nuclear, marine, airline, mining,
chemical, and petroleum industries. System analysis tools covered
include risk management, PHA, FTA, FMEA, MORT, PSM, and JSA.
