The sabbatical leave program requires that you give a report on your leave within three months of the end of the leave. Please send one copy of this form (and supplementary pages, if needed) to your departmental chairperson or school dean and one copy to the Dean of the Faculties. Describe your activities in some detail and show clearly the scholarly and professional benefits derived. Comment on any problems encountered which should be known by those who implement Indiana University's sabbatical leave policies. Reports on completion of sabbatical leaves are made available to the Committee on Sabbatical Leaves for use in evaluating subsequent applications.
