Gardasil HPV Vaccine
Gardasil is the first vaccine developed to prevent cervical cancer, precancerous genital lesions, and genital warts due to Human Papillomavirus (HPV). The Advisory Committee on Immunization Practices (ACIP) recommends routine vaccination for girls 11-12 years of age. The ACIP recommendation also allows for vaccination of girls beginning at nine years old as well as vaccination of girls and women 13-26 years old.

The company that makes Gardasil vaccine (Merck) has set up a program to offer free vaccine to LOW INCOME and UNINSURED patients. The IU Health Center does its part to assist our patients to apply for free vaccine but the final decision is in the hands of Merck. People who have health insurance or are not in low income situations will not be accepted into this program.
If you believe you qualify for free vaccine, you can get an application from the web site below or at the IU Health Center. Fill out page 1 and sign the application and hand it to an employee at the 2nd or 3rd Floor Clinic Desk or the nurse in room 216. We will provide page 2.
The IUHC will send the application to Merck and we will notify you by e-mail of the outcome within 3 working days. If you do not hear from us in 3 days, please call 855-7514 to check on the status of your application. When you know Merck’s decision, you may call 855-7688 to schedule an appointment for the vaccine. If Merck provides free vaccine, there will still be a small charge for administering the injection.
THE APPLICATION MUST BE SUBMITTED BEFORE YOU RECEIVE THE VACCINE AND MUST BE RESUBMITTED BEFORE EACH DOSE.
