Follow these steps to become a member:
1.) Download and fill out the correct membership form.
2.) Print the completed form and sign/date the last page.
3.) Photocopy the front and back of any current CPR/AED, First Aid, or EMT-B certifications you have. (EMTs must be certified in the state of Indiana.)
4.) Place the photocopies, your membership dues ($40 for the entire school year, $20 for a single semester), and your completed membership form in an envelope with your name clearly written on the front.
5.) Drop off all items at our office: Indiana Memorial Union, Student Activities Tower, Suite 672.
6.) If nobody is in the office when you drop off your envelope, please slide it under the door.
7.) Email digital copies of your current certifications to firstname.lastname@example.org with "Attn: Membership" in the subject line.
