Requesting a Waiver

Spring Waiver Deadline: January 21, 2010

If you have insurance from your home country, or other sponsor, that covers you while you are in the United States and would like to waive out of the IU mandatory health plan, you must do the following.

  1. Inform the Assitant Director of Client Services of your intention to be waived out of the IU mandatory insurance by filling out the online eForm on iStart.  If you have not received your network ID, then log in to “Temporary iStart Services” with the link at the bottom of the right-hand box on the iStart homepage.  In submitting your request, you will be required to enter information regarding your insurance company’s name, your policy number, and your dates of coverage.
  2. You will be asked to check whether or not your policy conforms to the requirements by checking the appropriate boxes.  All insurance must meet these requirements to qualify for a waiver.

Once you have verified that your insurance meets all the requirements, click “Submit” and your request will be reviewed and either approved or denied.

All requests must be submitted within 30 days prior to a semester beginning or within the first 10 days of classes.  No waivers will be granted after this time.  Waivers must be applied for on a semester-by-semester basis.
