If you have had a TB test done in the United States, please send the results to the IU Office of the Registrar, Franklin Hall 100, 601 E. Kirkwood Avenue, Bloomington, IN 47405. Otherwise, you must call the IU Health Center at 855-7688 to schedule an appointment for a TB test. Please wait until you know your class schedule before calling. When you call, press 1 from the menu in order to schedule an appointment with the Laboratory for a TB blood test. Please do this by May 30, 2008 for Summer I students, and June 30, 2008 for Summer II students.
Come to the IU Health Center at the time and date of your appointment, 600 N. Jordan Ave., at the corner of 10th Street and Jordan Ave. If you have any chest x-rays or health records from previous TB tests, please bring them to the appointment. The cost of the TB blood test is $60.00. Charges may be billed to your Bursar account. You will be given a statement about the charge to send to your insurance company to apply for reimbursement.
After registering for classes, you may go to one of the two Campus Card locations (Eigenmann Residence Center or the Indiana Memorial Union) to pick up your student ID during your free time. The Eigenmann Hall location is in the lower level next to the bookstore on the corner of Union St. and 10th Street.
