Departmental E-Form Request

About J-1 Exchange Visitor Status

In order to request J-1 visa certification for a new or current exchange visitor, the department must submit an electronic eform request or extension eform request through the iStart system.  Please note that the e-form should be submitted to the Office of International Services (OIS) at least three months in advance of the J-1 program start date at Indiana University in order to ensure that all documents needed to process the request are received and that the exchange visitor will have sufficient time to secure a visa at a US embassy or consulate abroad.

Departmental access to the iStart system can be requested via iStart.  After departmental access is granted, an email confirmation will be generated directing the department to the online e-form.
