CPT Check List

• Curricular Practical Training Application: Student Section

• Curricular Practical Training Application: Advisor Section.  Advisor should be sure to provide detailed answers to the questions asked at the bottom of the forms.

  Please submit the appropriate form(s):

  • Internship/Practicum form: if your position fulfills an internship or practicum requirement or option for your program of study
  • Regular Course or Independent study: if your position fulfills requirements for a course in your program of study
  • Material and Substantial Contribution to Thesis/Dissertation: if your position is necessary to complete your thesis/dissertation.
  • Integral Part of Established Curriculum/Degree Program for Doctoral Students: If your position fulfills a deficiency in your degree program.
• Job offer letter with all requirements included as per instructions on the OIS website.
• Registration in CPT-appropriate course for semester CPT is requested

Note:

Complete CPT applications take two weeks to process

• PD/AD/MM students: please read the specific information on the OIS website.
• Graduate students with on campus employment note: If you plan to work in another position on or off campus in addition to your current position, you must submit CPT applications for each position.

Note:

• CPT is granted for one semester at a time.  Therefore if you want CPT for multiple semesters you must apply for it each semester in advance of the dates requested.
