Counselor Signature Form

Our admissions application has a section to be completed by the student’s guidance counselor or advisor. Please:

  1. Complete the Counselor Signature Form (PDF). Students who apply online are instructed to print the form for you, or you can download it here.
  2. If you wish, send a letter of recommendation (this is optional and not required).
  3. Please list all courses the student is taking on the Counselor Signature Form or Electronic Secondary School Report if the student’s senior schedule is not included on the official transcript.
  4. Mail the form, transcript, and optional letter of recommendation to:
