Counselor Signature Form
Our admissions application has a section to be completed by the student’s guidance counselor or advisor. Please:
- Complete the Counselor Signature Form (PDF). Students who apply online are instructed to print the form for you, or you can download it here.
- If you wish, send a letter of recommendation (this is optional and not required).
- Please list all courses the student is taking on the Counselor Signature Form or Electronic Secondary School Report if the student’s senior schedule is not included on the official transcript.
- Mail the form, transcript, and optional letter of recommendation to:
