Please use the form below to request permission to reprint material from an Indiana University Press journal. Unless otherwise noted, all fields must be completed before the form will be processed. Please fill out a form for each article to be reprinted even if the articles are from the same journal, volume, and issue, and are intended to be used in the same publication.

PERMISSION WILL BE GRANTED FOR ONLY ONE PRINT VERSION OF YOUR PUBLICATION IN THE ENGLISH LANGUAGE. ANY OTHER USE REQUIRES ADDITIONAL PERMISSION.
