JOURNALS REPRINT REQUEST FORM

Please use the form below to request permission to reprint material from an Indiana University Press journal. Unless otherwise noted, all fields must be completed before the form will be processed. Please fill out a form for each article to be reprinted even if the articles are from the same journal, volume, and issue, and are intended to be used in the same publication.

Once your use of the material specified below is approved, a contract will be transmitted to you electronically. It will include all terms and the relevant fee information. If you agree to the terms, please indicate this by checking the appropriate box and returning the form to us.
