Acceptance Materials
As part of the acceptance process, students and guardians are required to complete several documents and send them to the IUHPFL. In addition, initial payments are required to secure the student's spot in the Program.
Accepted students should print and review the Instructions for Acceptance Materials & Payments to learn how, when and where to submit materials and payments. Keep this list on hand to refer to it regularly.
Files are best viewed in Adobe Reader v. 10.0 or higher. Download the latest version of Adobe Reader.
Required Documents
Each student submits several documents via mail and another set of documents via e-mail to iuhpfl@iu.edu. First download all materials to your computer. Students should be able to complete each form by utilizing each document’s form fields.
Students should select the Personal Information Form (PIF) reflecting their language. Only Mexico-bound students should complete and mail the Mexico Travel Permission form.
Note: MAC users must download all forms to their hard drive to avoid the “preview mode.” In preview mode, Adobe Acrobat will not allow forms to be filled in correctly. Other issues may include browsers incompatible with Adobe Acrobat, including Safari. If forms show blurry or black, applicants may want to switch to Firefox or Google Chrome.

Required Payments
Participating students make two types of payments: travel fees and Program fees. Recipient addresses for materials and payments are different. Due to Indiana University policies, materials and/or payments sent to the wrong address will NOT be forwarded to the correct recipients. Instead incorrect mail is returned to the sender and can result in delays and missing the Program deadlines.
1. Travel fees
For safety and organizational purposes, students travel as a group and accompanied by their instructors on prearranged group flights. Travel begins and ends either in Chicago or Indianapolis. Students are responsible to arrange travel to and from the airport. All travel arrangements and travel payments are made with Travel Leaders, the IUHPFL assigned professional travel agency.
Travel payments are due in two installments as listed under Program Fees & Funding and made directly to Travel Leaders. The first travel installment due date varies depending on whether the student was accepted outright or with interview.
2. Program fees
Program fees are submitted in three installments. For the exact breakdown, students should refer to the Program Checklist for Students. All Program fee payments must be made to the IU Accounts Receivable address.
Students should not send payments to the IUHPFL office. All payments will be returned and may delay a student’s acceptance status.
