Patron Confidentiality On-Line Form
The "Librarians" Code of Ethics' adopted by the ALA Council states the following concerning confidentiality of user information: "Librarians must protect each user's right to privacy with respect to information sought or received and materials consulted, borrowed or acquired." The Indiana University Libraries support and follow this policy.
The Chemistry Library Circulation Desk worker is responsible for upholding this policy. This means that when any patron, faculty included, asks the identity of another person who has a particular item checked out, reserves included, this information may not be released. We can tell you the date due, the status of patron, and/or recall the item.
EXCEPTION: Because books are often kept in the labs, and because people often only need to quickly consult the material, and because the Chemistry Department is basically a friendly place, we have allowed patrons to submit a Circulation Records Waiver. If you choose to submit this waiver, the library will release your name to the person needing to the use the book(s) and they will contact you. This may save you from having books which are regularly used in your labs from being recalled. This will also allow the person wanting the material to gain access quicker than the current process of mailing notices back and forth.
