Registration & General InformationOfficial Registration ChecklistAll teams advancing to the 2006 National Science Olympiad Tournament at Indiana University must have all forms filled out and submitted as outlined in the "Official Registration Checklist" below in order to compete. 1) Team Pre-Registration Form: Fill-out and submit the online pre-registration Form. This form tells us that you are coming and gives us pertinent information that we need to make this tournament a success. For teams advancing from State tournaments held in April '06, it is imperative that you fill this out immediately so we can include you in programs & materials that have early May print deadlines. 2) Determine which housing option is best for your team. Click here for housing packages & options. 3) Housing Information Form: Fill-out and submit the online Housing Information Form. This is required for all teams regardless of where you are staying. This will help us make sure we have enough hotel and/or on-campus housing available. 4) Housing Roster Form: Fill-out and submit the online Housing Roster Form. This form is only necessary for the teams that are staying on campus in IU Residence Halls. You will need a complete name, mailing address, grade, and birth date for every student when filling out this form. All teams staying in Halls of Residence MUST submit a housing roster. This roster allows you the flexibility of making individual room assignments; after all you know how to pair your students better than we do. 5) Talent Consent/Publicity Waiver: Print and make copies (enough for every teacher, student, parent, chaperone, etc.) of the Publicity Waiver. Pease note if someone from your team opts not to fill-out this form, our photography & video crews will avoid taking pictures of your entire team while on campus. Talent Consent Forms will be collected the day you arrive in Bloomington at the Team Registration area. 6) Medical Consent and Release from Liability Forms: Print and make copies (enough for every student) of these two forms. Note: NO team will be allowed to compete without these forms – so be sure you have one form per student attending. These forms will be collected the day you arrive in Bloomington at the Team Registration area. Registration will go faster if you have one set of forms per person stapled together and numbered in the right hand corner. 7) Code of Conduct: Print and have the entire team including coaches, competitors, alternates, and observers sign the code of conduct form. This form will be collected along with your medical consent and release for liability forms at the Team Registration area. 8) Final Registration Form: Fill-out and submit the online Final Registration Form. You will need complete name, mailing address, grade, and birth date for every student when filling out this form. Before submitting this form, please print out a copy, then obtain your principal's signature at the bottom. The principal's signature is to verify that all the students are from your school and are in the grade that you've indicated. Note: The paper copy with principal's signature, will be collected onsite along with the Talent Consent, Medical Consent, and Release from Liability forms. 9) Payment Options: Pay all fees (meals, housing, NSO keepsakes, Hoosier Hoedown and Banquet tickets).
