Overseas Study Advisory Council (OSAC)

Forms

Agreement & Release Form
Before participating on an IU program, students (and the parents of students considered dependents) must read and sign the IU Overseas Study Agreement and Release. This form details the obligations of both the program participant and Indiana University Office of Overseas Study.

Incident Reporting Form
Use this form to document the circumstances of incidents occuring during programs abroad.

Recommendation Form
IU instructors can mail, fax or complete recommendation forms online. Students can print out forms and instructions for their instructors.
