The Office of Overseas Study requires additional information of applicants. The forms below allow the collection of information that aid this office in providing the best services possible.
Agreement and Release
This form details the obligations of both the program participant and Indiana University Office of Overseas Study. Before participating on an IU program, students (and the parents of students considered dependents) must read and sign the IU Overseas Study Agreement and Release.
Australian Exchange Application
A limited number of exchange positions at reduced fees for study on IU programs in Australia (Adelaide, Canberra, Perth and Wollongong) are available to resident undergraduates with demonstrated financial need.
In addition to the exchange application, students must also submit an IU Overseas Study Application.
Evaluations
After your program is complete, use this easy online form to give your opinions and suggestions of your IU or Non-IU program.
Interview Appointments for Confirmed Applicants (Restricted Access)
Should your application be accepted for consideration and an interview be required, you will receive instructions by e-mail to sign up for an appointment time. Do not attempt to sign up for an interview until receiving notification!
Post-Acceptance Advising
After you have been accepted to a program, you will need to complete these forms and meet with your major advisor(s) and a study abroad advisor.
Recommendation Forms
IU instructors can mail, fax or complete recommendation forms online. Students can print out forms and instructions for their instructors.
