Updates to Birth Date
At times the Office of the Registrar will be made aware of errors on a student academic record regarding a birth date. This may be due to data entry or student error. The most common error is inverting the month and day numbers. August 10th could be entered 08/10/20xx or 10/08/20xx based upon student background (e.g., an international student or a student with a military background).
Current students whose birth date is incorrect (see the section below if you are a student employee ) should go to the Office of the Registrar and file a change of name form. Students will need to provide a photocopy of a legal document showing the correct date of birth. This corrected birth date will be reflected on the student’s official academic record.
Former students who are not currently enrolled or are no longer living in Bloomington may send their birth date correction request by mail, fax, or email. To insure that your records are complete, former students should also update their permanent (student home) address.
Required Documentation
For both current and formerly enrolled students the following documentation will be required to process your birth date correction request:
- A photocopy of the student's birth certificate, state driver's license, or passport
- - and -
- The student's signature
Please note the following considerations for formerly enrolled students making fax, mail and e-mail correction requests:
- Faxes must be legible in order to be received. Be aware that faxes tend to black out due to security features on some documents.
- Mail requests should include a student request letter as well as the photocopied birth certificate
- For e-mail requests, required documentation should be scanned and included as attachments addressed to regrdemo@indiana.edu. Please be aware that IT experts do not consider scanned documents to be secure.
Birth date correction requests should be sent to:
Indiana University
408 N. Union St.
Bloomington , IN 47405-3800
Fax: (812) 855-3311
regrdemo@indiana.edu
Changes generally occur on the same day the correction request is received (depending upon time of day and other requirements), but some IU-Bloomington computer applications may take an additional to 24 hours to update.
