Updates to Birth Date

At times the Office of the Registrar will be made aware of errors on a student academic record regarding a birth date. This may be due to data entry or student error. The most common error is inverting the month and day numbers. August 10th could be entered 08/10/20xx or 10/08/20xx based upon student background (e.g., an international student or a student with a military background).

Current students whose birth date is incorrect (see the section below if you are a student employee ) should go to the Office of the Registrar and file a change of name form. Students will need to provide a photocopy of a legal document showing the correct date of birth. This corrected birth date will be reflected on the student’s official academic record.

Former students who are not currently enrolled or are no longer living in Bloomington may send their birth date correction request by mail, fax, or email. To insure that your records are complete, former students should also update their permanent (student home) address.

Changes generally occur on the same day the correction request is received (depending upon time of day and other requirements), but some IU-Bloomington computer applications may take an additional to 24 hours to update.

Student Employees

Hourly Employees must complete a Personal Data Change Form and take it to the Human Resource Records and Benefits Office, Poplars E165. In all cases, Human Resources needs a photocopy of a legal document (birth certificate, passport, or state driver's license) showing the correct date of birth.

Academic Appointees (graduate assistant or associate instructor) must complete a Personal Data Change Form and take it to Academic Personnel Policies and Services, Bryan Hall 016. In all cases, Human Resources needs a photocopy of a legal document (birth certificate, passport, or state driver's license) showing the correct date of birth.
