Beginning 12:01 AM January 1, 2003 a new procedure is to be followed for on-the-job injuries at the Bloomington campus.
For the past several years the procedure for on-the-job injuries (and work-related illnesses) where critical care is not needed (i.e., an ambulance is not called) and during normal business hours has been to go to the Health Center for treatment.
Beginning January 1 employees (for those injuries and illnesses covered by workers compensation) shall go to the new Promptcare/Bloomington Hospital Center for Occupational Health (BHCOH) located at 326 Woodcrest Drive (the corner of East 3rd Street and Woodcrest Drive). This is a new building east of High Street. Look for the IU/Rebound entrance.
Effective January 12, 2003 these procedures also include non-employee student volunteers in laboratory settings for at least the initial visit after an injury in a laboratory setting. I.e., if both employees and student volunteers are injured in an incident in a laboratory setting it is no longer necessary to send some people to BHCOH-Promptcare and some people to the Health Center. All should go to Promptcare and the webform linked below should be completed for each person.
ORM claims personnel will handle the non-employee cases and will contact those injured about continued treatment.
The clinic's hours are 8:00 AM - 8:00 PM seven days a week.
When there is an injury/illness your supervisor must be notified and an authorization-to-treat form completed. This form can be completed on the web or can be downloaded and completed as a paper form. The supervisor (or responsible office) should send a copy of this form with the worker (for the initial visit) or complete the web form while the worker is being transported to the clinic. The Occupational Injury/Illness Report (that has been in use and required for many years) must still be completed for employees and should be faxed to Risk Management (855-9320).
If the web form is used Risk Management and the occupational health unit will receive copies. If the paper form is used a copy should be sent with the employee to the clinic and a copy immediately faxed to Risk Management.

Two required pieces of information when you submit the authorization form are your telephone number and your fax number. In some case one or both may actually be a departmental number rather than a direct line to you. The telephone number is required so the occupational health physician can contact you, if necessary, to discuss the employee's duties so they can better determine what work restrictions should be in place. The fax number is needed so they can immediately (as the employee finishes their visit) fax to you their job restrictions (if any). If a job description is not on file with BHCOH then a copy of the employee's job description should immediately be faxed to BHCOH.
We receive many telephone calls from employees who are not clear about what they need to do after an injury.
First, of course, is immediately report the injury to your supervisor. The supervisor will then give you instructions about seeking treatment.
When you have finished your visit at BHCOH you will be given (among other things) a slip of paper with your work restrictions. A copy of that slip is also faxed to your supervisor and a copy to us in Risk Management. Unless the physician has put you off work you immediately return to your department and report to your supervisor. If you have been given work restrictions your supervisor will begin the process of finding you restricted dutry.

A number of employees are on long term monitoring for health related reasons. If you are one of these employees:
You will receive a letter from Risk Management sometime in the next few weeks. This letter will have some information about the change in occupational health providers and a medical records release form. Within two to three weeks after you return the records release form to us you will receive a letter from the Bloomington Hospital Center for Occupational Health (BHCOH) scheduling your next monitoring visit. If you have a scheduled visit before you receive that letter, go to the Health Center as you have done in the past.
Other related occupational health services will also be provided at BCHOH. Examples include services required for respirator fittings and drug screenings. The offices responsible for arranging those services will notify the employees where to go.
