Informed Consent Statement

 

Mood and Sexual Interest: Questionnaires

Here is an information sheet that describes the study.  Please carefully read the entire form.

INFORMATION
You are invited to participate in a research study on the relationship between mood and sexual interest. Some individuals who choose to provide contact information at the end of the current study will be invited to participate in another paid study. The current questionnaires ask questions about your age, religion, marital status, HIV sero-status, alcohol and drug use, sexual history, your general mood, the effect that depression or anxiety typically has on your sexual interest, and your personality. Together these questions take about 30 minutes to complete.

RISKS
We believe there is little risk in participating in this study, but you will be answering questions about your sexual feelings and behavior, use of alcohol and drugs, and HIV serostatus. Your answers will be treated as strictly confidential. Because you will be answering sexuality related questions you should choose a private place to complete the questionnaire.

BENEFITS & COMPENSATION
You may find answering the questions interesting or even helpful as you explore your own feelings about these issues.

CONFIDENTIALITY
You will be asked at the end of the study if you would like to provide an e-mail address so that we may invite you to participate in future paid studies. If you choose not to provide an e-mail address, then no identifying information about you or your computer will be recorded. If you choose to provide an e-mail address, your identifying information will be treated strictly confidentially. If you are invited to participate in a second study, your identifying information will be kept until you complete or withdraw from the second study. If you are not invited to participate in a second study, the link between your identifying information and data will be destroyed within 30 days from when you complete these questionnaires. In all cases only the principal investigator will have access to the temporary link between your contact information and your data. No reference will be made in oral or written reports that could link you in any way to this study.

PARTICIPATION
If you decide to participate you are free to withdraw at any stage before completing the questionnaires. You are also free to refuse to answer questions.
