Reporting Other Awards

This electronic form is for reporting scholarships, grants, assistantships, fee remissions, or other assistance you will receive during the upcoming award period which are NOT listed on your Financial Aid Notification.

If you will be receiving an award from an IU department or school, there is no need to report that award on this form. The award will be added to your financial aid package by your department or school.

The receipt of a private scholarship may result in the reduction or repayment of need-based financial assistance previously awarded to the student by Indiana University. Whenever possible, self-help awards (loans and Federal Work-Study) are reduced before federal or institutional gift-aid.

Please refer to OneStart and click on the Financial Aid Office link if you have questions about financial aid. Allow up to three business days for adjustment to appear in OneStart.

If any of the awards posted below requires the Office of Student Financial Assistance to submit information to the donor (e.g. cost of attendance, federal aid received) in order for you to receive the award, please complete an Authorization Release form.

Please indicate the Award Title, Donor, Amount and Academic Term to which the award is to be applied (e.g. fall semester, spring semester, academic year, summer). It is important that you also forward the Reporting Donor Information form to all donor organizations for them to complete and include with the award check that they send to IUB. Please remember to report any renewable scholarships each year you receive them to be accounted for in your financial aid.
