Documents to Submit for Exempt Forms

For exempt studies (see Page 1 of the Application Packet for more info), you willl only need to submit:

  1. The cover sheet (Page 3 of the packet)
  2. The application
    1. Exempt Research Checklist (Page 4)
    2. Exempt Research Statement (Page 5)
  3. Your Informed Consent Statement (the page your students will sign)
  4. Also a good idea: a copy of the announcement to the students describing the study (it is important to phrase it clearly so that they know they do not have to participate)

Maintaining confidentiality is one of the major issues that arises in SOTL projects. One very effective mechanism to do this is to have a third party (usually a colleague or consultant from ISS) collect the consent forms and other project materials and hold them until after the final grades have been submitted.

Here is one example of an exempt proposal, including a first draft, required changes, and final draft.
