Tape Transcripts and Counseling Session Notes:

All students will video tape their performance with clients for review as a part of this class. If facilities are not available at your site special arrangements must be made by you to work with an additional client here at IU so you will have taped material for class. If for any reason the video on site or here at IU does not work you are to maintain an audio back-up tape of all your client work.

Students will review each tape and code them using the Hill observation system. Students will be expected to review and code the tapes before the class supervision sessions. Students will demonstrate proficiency on the Hill Observation system by coding a training tape to the point where there is a .85 level of consensus between their coding and the master coding of the tape. When tapes are presented to the class or used in individual supervision they will also be coded by reference number including the start and stop times of the segments to be reviewed. These reference numbers are to be written legibly on a separate sheet of paper so that another student can cue the tape while you discuss the content with the instructor and the class. These tapes are an important part of the class. Through these tapes you will demonstrate how you have changed your therapeutic style.

 Professional Case Notes:

All Students will be expected to maintain a copy of client assessment and counseling notes in the Center for Human Growth Data System. The operation and use of the data system be discussed during orientation. Students are responsible for learning how to use the data system of class time. Students are encouraged to keep complete and up to date assessment information and counseling notes since this material is a significant part of the final grade.

Students are to create a new folder for each client or group to which they are assigned. Each subsequent session is to be a reply to the client or group note. Instructor's comments will be interspersed in the replies. Each student must log the notes on the previous client session before a subsequent client session is initiated.

S.O.A.P.I.E. NOTE FORMAT

Therapy notes will be kept in the S.O.A.P.I.E. format.

 <S>ubjective impressions:

Client x is a clean, very proper looking woman. She is an R.A. and she carries a kind of controlled responsible look around with her. She's a somewhat together person. Although during the session she seemed a little confused, not knowing what her role was apparently.

 <O>bjective impressions:

She seemed tense in the way she was sitting. Erect, with her hands in her lap. Her eyes seemed to rarely make contact with mine. She smiled a lot.

 <A>ssessment:

 Obsessive Compulsive Disorder

 <P>lan:

My Plan for Client x is to observe how her obsessive thoughts surface. I want her to take responsibility between sessions for her thoughts.

 <I>ntervention(s):

The interventions I would use include confrontation , interpretation and reassurance.

 <E>valuation:

Our session was an exciting one for me, because I found Client x asking and hinting several times for me to stop her from becoming compulsive. I felt somewhat uncomfortable with her demands, but I continued to focus the discussion on her and her responsibilities. I think her dependency on me could become an issue to work on (is this some sort of transference?)

 Required Texts:

 None.

 On Site Meetings

The students will be responsible for arranging a time on a Monday for a meeting between the instructor, student and the on-site supervisor.
