Change PERF Investment Direction

The PERF Retirement Plan consists of two parts: a pension plan (a defined benefit provision) and an annuity savings account benefit (a defined contribution provision).

Participants may invest their annuity savings account in one or more investment funds, and they can change their allocation once per calendar quarter to become effective the first day of the next calendar quarter (with 30 days notice).

1. Review the information at the PERF Web site to learn more about your benefits.
2. Change or review investment options:
   • Complete a PERF Investment Direction form, or
   • Log onto www.perf.in.gov to access your account
3. File the form with PERF at the address below.
