Divorce or Legal Separation
Following a divorce or an annulment, you will be able to--and in certain cases, you must--make changes to benefit plans outside of Open Enrollment.
If your former spouse or stepchildren are enrolled in the IU dental plan and/or medical plan, they are no longer eligible and you must remove them.
For changes to marital status, name, address and emergency contact.
To make changes to any of the plans listed immediately below, you will need to
complete
the Change of Status form and
the subsequent personalized benefit enrollment form. Step-by-step instructions appear on the respective plan pages below. You must make the change within 30 days of the divorce or annulment date.
A copy of the marriage certificate, and if you are dropping IU coverage, documenation of enrollment in your spouse's plan(s)will be required.
- Remove your former spouse or stepchildren from your IU dental plan.
- If you are currently covered under your spouse's dental plan, enroll yourself and your children in the IU dental plan.
- Remove your former spouse or stepchildren from your current IU medical plan.
- If you are currently covered under your spouse's medical plan, enroll yourself and your children in an IU medical plan.
Personal Accident Insurance plan
- Add or drop coverage.
- Increase or decrease the coverage level.
Health Care:
- Decrease election if consistent with change.
Dependent Care:
- Add or increase coverage if the divorce creates or increases need for dependent care.
- Drop or decrease coverage if the divorce decreases or eliminates the need for dependent care.
(Consistency rule is satisfied if change corresponds with change of status that affects either eligibility for coverage under Plan or eligibility of expenses under applicable IRS regulations.)
You may also want to consider changing your designated
beneficiary for applicable plans. Such changes
do not require completion of a change of status form and
can be done at any time.
Change Beneficiaries on Retirement and Insurance plans
- Change the beneficiaries on your base retirement and life insurance, as well as any other supplemental plans.
IU Retirement Savings Plan 457(b)
- Begin, change, or stop salary deferrals.
IU Tax Deferred Account (TDA) formerly IU Tax Deferred Annuity Plan
- Begin, change, or stop salary deferrals.
