2008 Medical Care Plans Blue Preferred Primary POS
Medical Services
This plan is available to eligible Indiana University employees, who reside in the Indianapolis, South Bend, Kokomo, New Albany, Bloomington and Louisville areas. When services are provided by or referred by a network primary care physician (PCP) or a Blue Preferred network provider, members receive full benefits. Partial benefits are received for non-network services. Members must select a primary care physician at the time of enrollment.

Prescription Drugs (Rx)
This plan’s prescription benefit includes medications available on the Anthem Formulary, which may offer potential savings when a physician prescribes formulary medications.

Blue Preferred Primary POS offers full benefits from Anthem Behavioral Health network providers. Partial benefits are received from non-network providers. All services must be authorized in advance in order to be covered.
