Medical Plan Types
Indiana University offers a choice of three medical care plans. All IU-sponsored plans are comprehensive in nature covering medical, prescriptions, behavioral health, and transplant services. None of the IU-sponsored medical plans exclude pre-existing conditions.
IU PPO $900 Deductible Plan (more information)
This is a PPO plan that offers a large network of physicians and hospitals—the Anthem Blue Access network. The network provides access to quality care and saves money through Anthem’s negotiated reimbursement rates. You do not have to select a Primary Care Physician (PCP) to manage care. If you go to in-network (preferred) providers, the plan will pay higher benefits. If you use an out-of-network provider, the plan will pay lower out-of-network benefits. IU pays 90% of in-network medical costs once you reach the plan deductible. Your copay is limited by an out-of-pocket maximum—when your payments reach the maximum there is no copay. Preventive services are covered—no deductible and a 10% copay. The deductible applies to all other medical expenses except prescriptions, transplant services, and behavioral health.
Blue Preferred Primary POS Plan (more information)
For this plan, you select a PCP (primary care physician) to help manage care and use the Anthem Blue Preferred network which includes a cost-effective laboratory network. The plan includes in-network and out-of-network benefits like a PPO plan. When you use the designated PCP or network providers you pay no deductible and have lower out-of-pocket expenses. If you use an out-of-network provider, the plan will pay lower out-of-network benefits. The plan has fixed-dollar copays for most in-network services including preventive care. Copays are limited by an out-of-pocket maximum—when your payments reach the maximum there is no copay. This plan has a drug formulary that determines the brand copay level. Non-formulary drugs are covered, but you pay a higher copay. This plan also has a laboratory network that must be used for the highest level of benefits.
IU HDHP PPO & Medical Savings Plan (more information)
This plan is somewhat different than traditional medical plans--it includes both comprehensive medical coverage (IRS-qualified High Deductible Healthcare Plan (HDHP)) and a tax-advantaged savings account.
