Life and Accidental Death & Dismemberment Insurance

Plan Provisions


Basic Life and Accidental Death & Dismemberment insurance is available to all eligible Graduate Appointees and Fellowship Recipients. The policy is underwritten by Anthem Life Insurance.

Plan Contributions

Basic Life and Accidental Death & Dismemberment insurance is available to all enrollees participating in medical and dental coverage. Those who are eligible are automatically enrolled in the policy; no action is required for coverage. The policy is underwritten by Anthem Life Insurance.

Initiation of Coverage

Termination of Coverage

Coverage terminates when appointment with Indiana University terminates or the fellowship ends at the end of the semester. There is an option to convert the life insurance policy when no longer eligible for the plan.


It is recommended that those who are eligible complete the Life Insurance Beneficiary Form (PDF) to designate beneficiaries under the policy. Completed forms should be returned by fax or mail to University Human Resources:
