Enrollment Forms

These are PDFs that you print, complete, and return to your campus Human Resources office.

To enroll in a plan an employee must complete all three of the following steps:

  1. Complete a salary reduction or deferral agreement.
  2. Establish a Plan account at an authorized investment company by completing an account application.
  3. Return the completed forms to the campus Human Resources office no later than 30 days prior to the next pay date. (If both documents are not returned to the campus human resources office, salary deferrals cannot be made to the Plan.)
