Open Enrollment Checklist

Open Enrollment:  November 10 - 21

Open Enrollment is your annual opportunity to change your benefit elections for the next year. IRS regulations require that your Open Enrollment benefit elections remain in place for the entire year unless you experience an IRS-defined Family Status Change. A change in status, such as marriage, divorce, or the birth of a child allows a corresponding change to your elections, but only within 30 days of the event.

All benefit elections made during Open Enrollment will be effective January 1, 2015.

Review the open enrollment information. Consider any changes you wish to make.
Attend one of the Open Enrollment info sessions or webinars or contact if you have any questions.
Review your existing medical and dental benefit elections. If you need to change your elections, complete the online open enrollment form in the Benefits Self Service area of OneStart.

Instructions to Enroll

  1. Log in to OneStart at
  2. Click on the Services tab.
  3. Click on the Employee Center link on the left-hand side of the screen.
  4. Click on Benefits.
  5. Click on the Open Enrollment button under Benefit Actions.
  6. Enter benefit selections for each plan using the online form.
  7. Click Submit after making all applicable changes to send your selections to University Human Resources.
  8. Check your IU email account for confirmation of your selections
  9. When done, especially on a public computer, always remember to log out.
If enrolled in the Health Savings Account (HSA), you must re-enroll in the HSA . Your HSA election will not rollover into the new plan year. You can make your 2015 HSA election during open enrollment, or at any time during the year.  You can choose to enroll either of the HDHP Medical plans and still waive the HSA if you are not eligible to make contributions to an HSA.
Complete the Tobacco-free Affidavit if you are not a tobacco user or will not use tobacco during the upcoming year.  Even if you are not changing medical coverage, you must re-enroll in this benefit each year.  Find the affidavit at the bottom of the medical page of the online open enrollment form.
Determine if you want to elect the TSB Health Care and/or Dependent Care Reimbursement Account(s). If yes, complete the TSB section of the online open enrollment form. You must re-enroll in this benefit each year.
Review your Personal Accident Insurance elections. You can add, change or drop coverage.
Submit your elections. After making benefit selections online, be sure to submit them and receive the confirmation email with your 2015 elections. Until you click ‘Submit’, your elections will not be registered for 2015.
Carefully review your confirmation email to be sure you and your family members are enrolled in the selected benefits.
If you are adding a dependent for the first time be sure to turn in required dependent eligibility documents.
Make sure you have completed your open enrollment selections by Friday, November 21, 2014.
In January, complete the TSB direct deposit form(s) and the HSA beneficiary information at


Note:  You can change and resubmit your enrollment elections as many times as you need to until the end of Open Enrollment on November 21st. With each submission you will receive a new email notice confirming your benefit elections. Please review each notice. Changes to your benefit elections cannot be made after the close of Open Enrollment.
