Vision Wear Plans

Anthem Blue View Vision

Those enrolled in an IU PPO medical plan (PPO HDHP, PPO $900 Deductible, and PPO $500 Deductible), will receive the Anthem Blue View Vision benefit with no separate enrollment. Membership in this plan will be identified on the member’s Anthem medical plan card.

EyeMed Vision

Beginning January 1, 2015, those enrolled in the IU Health HDHP plan will receive the EyeMed Vision with no separate enrollment.  A separate membership card for EyeMed will be issued to members by mail or you can view a copy of your ID card by downloading the “EyeMed Members” app through the Apple iTunes store.

Finding a Network Provider

In-network providers include the IU School of Optometry Eye Care Centers (Atwater Eye Care Center and Indianapolis Eye Care Center), LensCrafters, Pearle Vision, Sears, Target, 1-800 Contacts, and other independent eye care professionals. To find an independent in-network provider, follow the steps below:

Anthem Blue View Vision
(PPO HDHP, PPO $900, PPO $500)

  1. Visit www.anthem.com.
  2. Click on Find A Doctor under the “Useful Tools” section on the right side of the page.
  3. Select Indiana under the state, Blue View Vision under “Search by selecting a plan/network.”
  4. Click on the Select and Continue button.
  5. Enter a Zip Code or City and State under the “Located near.” Select a distance to provider, if desired.
  6. Click the Search button.

EyeMed Vision
(IU Health HDHP)

  1. Visit www.eyemed.com.
  2. Click on Find a Provider in the green menu bar at the top of the page.
  3. Enter your Zip Code to find providers nearest you.
  4. Select INSIGHT in the Select Network drop down menu.
  5. Click the Get Results button.
  6. Or download the “EyeMed Members” app through the Apple iTunes store on your iPhone, iPad or iPod Touch.
  7. Or call 1-866-804-0982 for assistance.

Note: Medical eye exams, such as for treatment of glaucoma or retinal detachment, remain a covered benefit under medical plans; however, they will be subject to the medical plan deductible and co-insurance.
