About the file formats
PDF (portable document format): PDFs can be opened using the free Adobe Acrobat Reader program. They must be printed, completed by hand and then return as directed. Not recommended for applicants with ADA accessibility issues.
RTF (rich text format): This format can be opened and completed with any word processing program: MS Word, Word Perfect, AppleWorks, etc. The completed form can be returned by e-mail or printed and returned as directed.
HTML - printable: The printable HTML form is to be completed online then printed and returned as directed.
HTML - online: Available upon request. This HTML form can be completed and submitted online. Employment Services does not supply technical support of this form. Departments must have a web developer with scripting (ColdFusion, Perl, etc.) and database skills to use this form. If your department meets these requirements please contact Employment Services to obtain the HTML template.
How to download
PDF (portable document format): Make sure you have the free Adobe Acrobat Reader installed on your computer. If your browser asks what to do with the PDF file when downloading choose either "open with..." Adobe Acrobat Reader or "save to disk".
RTF (rich text format): If your browser asks what to do with the RTF file when downloading choose either "open with..." MS Word or another word processing program or "save to disk".
HTML: Open the page, go to:
- File > Save (Page) As;
- choose a location to save the file;
- name the file;
- save as type: "Web Page, complete";
- click Save.
