About the file formats

PDF (portable document format): PDFs can be opened using the free Adobe Acrobat Reader program. When using the most recent version of Reader, PDFs can be completed digitally, saved and emailed. PDFs can also be printed and completed by hand.

RTF (rich text format): This format can be opened and completed with any word processing program: MS Word, Word Perfect, AppleWorks, etc. The completed form can be returned by e-mail or printed and returned as directed.

HTML - printable: The printable HTML form is to be completed online then printed and returned as directed.

How to download

RTF (rich text format): If your browser asks what to do with the RTF file when downloading choose either "open with..." MS Word or another word processing program or "save to disk".

HTML: Open the page, go to:
      - File > Save (Page) As;
      - choose a location to save the file;
      - name the file;
      - save as type: "Web Page, complete";
      - click Save.
