I. Evaluation of performance.

(To be initiated by the staff member; the supervisor adds his/her comments) Please comment on your significant accomplishments for the appraisal period. You should use duties from your job description, specific goals and objectives set at the beginning of the appraisal period and/or any subsequent relevant documentation as the template for documenting your achievements. Please include any special awards, activities or recognition during this evaluation period.

II. Summary of performance strengths and development needs

The supervisor completes this section, commenting on narrative in the sections above, input from feedback questionnaires, and documentation of observations made throughout the appraisal period.

III. Goal setting.

Supervisor and staff member together should define and plan objectives for the next appraisal year.

After UITS staff and their supervisors have discussed this performance evaluation and development plan and have agreed to the final appraisal, this form should be signed by staff, supervisor, and the appropriate Director or AVP and placed in the staff member's UITS personnel file.
