• Please enter the four pieces of information about the staff member you are evaluating provided to you in the email message.

• Use the Tab key to move from box to box.

• Take a few moments to read each of the behavior statements which follow and select the number that you feel best applies based on your knowledge of the individual's performance.

• The only optional field is the comment section. Each statement must have a response, so please enter 0 if no opinion/not applicable.

• We appreciate any comments you may wish to add at the bottom of the form. Specific comments for a rating of less than 3 will facilitate improvement.

• When you have completed the questionnaire, select the submit button to have it entered into our database.

• You will receive immediate confirmation that your entry was successful, or a message indicating that something was not completed correctly.
