Release from Liability Templates
OVPGC works with all units and departments in creating “Assumption of Risk and Release from Liability” Agreements to be signed by participants of various University-related trips, programs, and activities. These Agreements serve to minimize legal exposure to the University in the event of injury or property damage to any participant. It is important that the Agreement you use is unique and specific to the particular trip, program or activity planned, and that you provide each participant a chance to review the Agreement and ask questions prior to signing. For participants under age 18, a parent or legal guardian will also need to sign. Please retain originals of the Agreement with your files.
To help you prepare your Agreements to have them ready for participants to sign in advance, OVPGC provides below activity specific templates for you to complete and submit to our office for review. Please use the appropriate template, adding in the program specific information where designated (e.g. dates, times, description of activities, and specific risks related to those activities). Please submit the completed template as a Word document to our office at the following email address: email@example.com.
Whenever possible, please submit your completed template for review two weeks in advance of the activity planned.
