Sexual Misconduct, Drugs and/or Alcohol: Tracking Campus Student Education Programs & Initiatives

PURPOSE: This form is designed to collect information on all student programming related to sexual misconduct, drugs and alcohol offered for the past academic year. The data will be used for a number of purposes, including tracking, internal and public reporting on prevention efforts (including the Annual Security Report), evaluation and identification of gaps.

INSTRUCTIONS FOR COMPLETING THE FORM:
- Please fill out the form for each unique program or initiative offered that included some component of sexual misconduct, drugs and/or alcohol prevention education.
- If your program or initiative was offered more than once, or repeated over time, you can submit ONE form and include the total number of participants in the form (e.g. indicate you hosted 20 bystander intervention presentations over semester with a total of 500 participants; or 10 bulletin boards posted regarding consent and alcohol in residential housing reaching 200 residents).

- The fields below will prompt you for the information needed.
- The term "programs and initiatives" is broad and includes any program, event, course, training, outreach, informational tabling, activity, online programming, presentation, speaking engagement, or other session that covers sexual misconduct issues.
- Sexual Misconduct is a broad term that includes sexual harassment, sexual assault, sexual violence, dating and domestic abuse, and stalking.
