PRE-OPTOMETRY REQUIREMENTS WORKSHEET FOR ADMISSION

GRADES OF C- OR BELOW CANNOT BE USED TO FULFILL PRE-OPTOMETRY REQUIREMENTS. College grades of "S", "P", and "CR" cannot be used to fulfill pre-optometry requirements, but can be used to fulfill remaining electives required to meet the 90 hour minimum. AP credits are acceptable. Each course can fulfill only one requirement with one exception*. For example, an introductory microbiology course cannot be used for both the microbiology requirement and the advanced biology requirement. NOTE: The minimum hours must be met or exceeded.

It is preferred that the Supplemental Application, including the Indiana University eApp and the Optometry Departmental Questions, be completed and submitted at the same time as the OptomCAS application. Please submit the Supplemental Application no later than February 1st.
