What is minimum necessary standard?

The August 14, 2002 version of the HIPAA final privacy rule states the Minimum Necessary Standard applies when using or disclosing protected health information (PHI), or when requesting PHI from others, a covered entity must generally make reasonable efforts to limit itself to "the minimum necessary to accomplish the intended purpose of the use, disclosure, or request."
